Langley High School Fund Raiser Request "New 2009-2010"

· Use this form for any fund raiser request for your activity, club, organization, sport, etc…

· This helps us make sure that there are no “duplicates”.

Langley High School Facilities Reservation & Service Request Form (LHS FRSRF)

· Use this form to reserve a room, tables, chairs, technicians, lcd, etc… for an event (during or after school).

· A community use application is required in addition when use is for evening or weekends.

· Complete for evening and weekend events; in addition to the LHS FRSRF form

· Also, print the R8420 page for your information.
